BACKGROUND INVESTIGATION RELEASE
Having filed an application for employment or, if hired as an employee with JW Marriott Las Vegas, or a subsidiary, do hereby authorize JW Marriott Las Vegas to seek from school officials, government agencies, the military, law enforcement agencies, credit reporting agencies, present or previous employers if so indicated above, any and all information in their knowledge or possession pertaining to my employment history or my qualifications and ability to work at JW Marriott Las Vegas, including but not limited to , salary history, attendance record, character, academic record, credit history, ability to perform the essential functions of my prior positions and any performance, behavior, attitude, or other problems or good points perceived by them. This information is for record keeping purposes and may affect employment decisions. This questionnaire will be kept separate from your application and any subsequent file.

Further, I authorize JW Marriott Las Vegas to seek from any agency, including but not limited to, the Nevada Highway Patrol, the Las Vegas Metropolitan Police Department and any other agency of criminal justice relied upon by JW Marriott Las Vegas, to release any criminal record information concerning me, including, but not limited to, the results of and reports concerning any investigations, and any and all documentation, test results or information of any type obtained from any source during the course of such investigation.



PRE-EMPLOYMENT DRUG TESTING PROCEDURE CONSENT
I further acknowledge that as a requirement for consideration for a position with JW Marriott Las Vegas, I must submit to a Pre-Employment Drug Testing Procedure at an independent testing laboratory within 48 hours of notification by JW Marriott Las Vegas. If I choose not to submit to the Pre-Employment Drug Testing Procedure, I will be automatically disqualified as an eligible applicant for employment at JW Marriott Las Vegas. I further understand that if my test results are positive, I will not be eligible for any positions at JW Marriott Las Vegas and the JW Marriott Las Vegas is not obligated to conduct another drug test on me for three months. Additionally, I understand that if my test results are negative, that does not guarantee me an offer of employment but only qualifies me to be considered for employment at JW Marriott Las Vegas. I further agree to the release of the test results to JW Marriott Las Vegas (and / or to a designated agent of JW Marriott Las Vegas) and that the results may be used as grounds for denying me employment with JW Marriott Las Vegas.

All documents pertaining to the Pre-Employment Drug Testing Procedure belong to JW Marriott Las Vegas, and JW Marriott Las Vegas is not obligated to provide any said documents to me.
